      For Patients with Kaiser Permanente Health Insurance       


Exceptional Voice, Inc. is an outside contracted provider for Speech-Language Pathology services for Kaiser Permanente.

    • Step-by-step exercises.
    • Listening exercises.
    • Structured feedback about speech and voice patterns.
    • A vocal hygiene program (“rules of good voice care”). 
      Breathing exercises.
       
FEES & BILLING POLICIES

Your physician has prescribed speech (voice) therapy for your problem.  We will directly bill Kaiser Permanente for your speech therapy. You will not receive a bill from us unless there are additional fees assessed to your account that Kaiser does not cover (co-payments, missed appointments, late cancellations).  We accept cash, checks and credit card payments (Visa, Master Card and Discover Card).  All credit card charges are processed via a virtual terminal.  We do not swipe your card, but write down your credit card information to use later to upload into the virtual terminal provided by a credit card processing company.  Your credit card information is then shredded. We do not keep a permanent record of your credit card information.  If you would like a receipt of your co-payments please ask.

 

Medicare will NOT pay for speech-language pathology services delivered at independent practice facilities (although recent legislative changes will be in effect in July 2009).  Exceptional Voice, Inc. will not bill Medicare for any speech/voice therapy charges.  Exceptional Voice, Inc. does not participate with Medicare or Medicaid.

 

A $65.00 fee will be billed if you do not cancel within 24 hours or miss your scheduled appointment.  It is our goal to provide the best possible care and to treat all those who seek our help.  Same day cancellations and missed appointments make it impossible to us to schedule someone on the waiting list. 

 

HIPAA (Health Insurance Portability and Accountability Act) regulations pertaining to transactions, privacy, and security apply to speech pathologists and audiologists who electronically bill insurance companies for their services or use a clearinghouse to do so.  Please read and sign our HIPAA Privacy statement.
